User :   my wife is 56 and in XXXXX

User :   she had an accident on the road while walking, a scooter rammed into her ankle and it fractured,
User :   by an XRay she was able to tell
User :   she lives 500 kms from a city
User :   a 9 hour car drive

User :   is it recommendable to drive with a fracture and no treatment?
Doctor :   so has she had any fractures ?
User :   10 years ago she had a rib fracture which completely healed
Doctor :   did she have a fracture this time as well
User :   yes
User :   in the ankle bone

Doctor :   So has a plaster cast been placed ?
User :   no
User :   no treatment has been done
Doctor :   when did this accident occur ?
User :   wednesday afternoon india time
User :   so far 18 hours and counting
Doctor :   ok, incase there is a fracture, then she can immobilise the foot by a plaster
User :   there are no proper hospitals in the area she lives
User :   only nursing homes and alternative clinics
Doctor :   Any general physician would be capable of placing PLASTER CAST on the limb

User :   requesting a nearby doctor to do it would be good?

User :   after that can she travel to a hospital for further investigation?
Doctor :   you can use new paper or hard cardboard sheets as a support around the hurt leg and wrap it with a thick rope

Doctor :   for pain reduction, take combiflam as and wheen needed

User :   after temporary cast u suggested can she travel for 9 hours? in a sleeping or lying down position by car?

Doctor :   since its a injury to the lower end of the foot / ankle
Doctor :   its not going to be any complications unless there is ongoing blood loss due to an external wound as well

User :   and i have one last question, are there chances of gangrene?
Doctor :   so she can be moved to the nearest medical center easily
Doctor :   if the injury is so severe that there is severe untolerable pain, discoloration of the foot to blue or black, with loss of sensation, or buring sensation, it means there is reduced circulation, and can lead to cancer

User :   I will advice her to do so
Doctor :   but a simple fracture is not comonly associated with gangrene
User :   to first get a cast by any doctor and travel to a good hospital
User :   ok
Doctor :   once you feel the pulse at the side of the foot and behind the ankle
Doctor :   it means the blood clow is intact and nothing to be worried about gangrene

Doctor :   warmth of foot, good pulse, no sensory changes or no numbness, and less amount of pain - are good signs

Doctor :   yeah even that will suffice for first aid and cast placement
User :   that hospital does not admit people without waiting and queue and emergency

Doctor :   u mean its a govt hospital ?
User :   an XRay was taken in that hospital where she found it was a fracture
User :   no
Doctor :   so doesnt that hospital provide even cast placement in the ER ?
User :   The Govt Hospital is demanding the accident report, which is not practical to obtain very soon
Doctor :   i understand the legal issues take time
User :   No, cast placement also is not given since it can be given in Govt hospital
User :   yes, the legal issues are postponing the treatment

Doctor :   and am sorry to know that
User :   thats why i recommended her and my sister to take her in car to
User :   which will take 9 hours slowly travelling
Doctor :   ok, that would be good idea and overnight journey will not cause much trouble as well
