User :   My father has aggressive lung cancer, congestive heart failure and kidney failure, he has been told that there is no more the doctors can do, he has come home from hospital, refusing medications and in the past 12 hours, has become very short of breath, increased swelling in the legs, is a grey colour, and is very cold, he is also experiencing pain around the kidneys, he is not eating much and is not drinking much, is it likely that the end is upon us?

Doctor :   the symptoms (very short of breath, increased swelling in the legs, is a grey colour, and is very cold, he is also experiencing pain around the kidneys) suggest severity of problem
Doctor :   rush him to emergency room
Doctor :   he will need urgent medical care to help him
Doctor :   is he on any chemotherapy or radiotherapy?
User :   that is what I thought, he will not go back to hospital he wants to die at home, just want to be prepared for what to expect
User :   he has not had any treatment for the cancer as it is far too advanced
Doctor :   ok. then his symptoms suggest that he is nearing his end, if he wants to be at home and die at home, see if medical help can be given at home
Doctor :   with his doctor and a nurse to take care of him along with family members around him
User :   I am a nursing student and giving him as much support for him, how long given the symptoms do you feel, before he really declines, and death is iminent
Doctor :   we cannot say, he may pull on for few hours, few days or even 1-2 weeks, depending on hid determination to fight back
Doctor :   his symptoms suggest overload on heart and possibility of kidneys failing
User :   last thursday he was in hospital, he said he was not feeling well, in the morning and within an hour, he had crashed and was revived and placed on life support, he was looking the same as he does now on the Wednesday, could it happen as quick as that again
User :   under normal circumstances he would fight back but he has accepted his fate
Doctor :   yes, it could happen again, he might need to be revived and may need ventilator support again
User :   they told us that they could not give him ventilator support anymore, that he is palliative, and he does not want any heroic efforts made to keep him alive, so do we just let him go when his level of consciousness diminishes
Doctor :   but sice he has accepted his fate, he might not pull on with teh support too
Doctor :   be with him as a family and give him emotional and mental support
Doctor :   such that he has a death he wishes for in his home
User :   thank you so much and I will continue to give him the support he needs and monitor his condition
