Doctor :   hi
User :   Hello
Doctor :   Dr here
User :   What does it sound like to you
Doctor :   may i know any other medications that u r taking ?
User :   Prilosec , and Tramadol
Doctor :   it definitely looks like acid reflux
Doctor :   and apppears due to laxity of the lower end of the esophagus
Doctor :   can u tell me what is the immediate step after u have your meals ?
User :   Yeah they found that , but the problem is , the pressure supposedly had gotten allot better , but the pain and problems have not ??
Doctor :   did they do any barium study ?
User :   Yes. also an esophageal manometry , and 24 hour ph study
Doctor :   what were the results ?
Doctor :   was surgery suggested ?
User :   They said my lower sphincter pressure was at about 1.5 ! But then a year later it got better. its up to an 8 now ? thats why they refused me for surgery. Thats why im frustrated
Doctor :   ok
User :   I still have they issues ?
Doctor :   ok
User :   if the pressure got better than why have i not felt better ??
Doctor :   now acid reduction is done by prilosec
Doctor :   can u tell me the dosage you take ?
User :   20.6 MG
User :   2 times a day
Doctor :   ok
Doctor :   do u use any antacids ?
Doctor :   and may i know your height , weight ?
User :   I was on a prescription antacid for years but it still did nothing i am 6"1 and 180 pounds
Doctor :   have you been tested for H pylori infection?
User :   no. whats that
Doctor :   H pylori infection of the gastric mucosa can increase gastric acid secretion
Doctor :   this is a chronic infection causing severe gastritis and resultant reflux
Doctor :   and symtoms that are resistant to medications
User :   How does that happened to someone ? if i do have it
Doctor :   it is a very common infection , occurs via ingestion
Doctor :   the bacteria can be killed by a course of antibiotics for about 3-4 weeks
User :   Could that be causing all of this ? im not sure how my valve could have been at a 1.5 and go back up to an 8 ? its all very confusing for me
Doctor :   along with better acid reducers like nexium
Doctor :   yes ,
Doctor :   it can cause such symptoms
User :   Thats going to be hard without insurance , but i will figure it out i guess
Doctor :   i would like to advise you to keep a upright position 30-45 min after meals so that the food gets time to move down the bowel
User :   Are you saying Prilosec is not good ??
Doctor :   lifestyle changes, diet changes can help very much
Doctor :   there are better acid reducers now,
Doctor :   prilosec may not be effective in some
Doctor :   all new medications are developed from omeprazole
User :   What over the counter meds do you suggest. Some of them tear up my abdominal
Doctor :   as of now, only omeprazole is OTC
User :   Thats what Prilosec is
Doctor :   nexium , rabeprazole are prescription meds
Doctor :   yes it is
User :   why do u thing my collar bone hurts ?
User :   is it related ?
Doctor :   no
Doctor :   its not related
Doctor :   it could be due to some other musculoskeltal pain
User :   it hurts on the same side as my throat ??
Doctor :   it may be related to some inflammation
Doctor :   try some OTC analgesics cream over there
User :   Thanks for your help. Have a nice day.
Doctor :   you are welcome
Doctor :   take care bye for now
