User :   Hi, I am in a remote foreign country and just ruptured my Achilles tendon playing tennis. A doctor here diagnosed it and I'm pretty sure he's right from what I have read. I am due to leave in 2 days, and fly back to the (through and). He will bandage it for me, but they don't have casts here. He says surgery probably isn't necessary. My questions are: 1) Can I wait to get back to the to get a second opinion on surgery vs non-surgery?, 2) Should I try to get a cast during my stopover in or can it wait until I get back? 3) I am flying business class, but are there increased risks of DVT because of the trauma to my ankle? Thank you.

User :   Ok, I am leaving in 2.5 days, but it takes more than a day to get back.

User :   I will be in for about 12 hours, but I could delay my flight if it is a better idea to see a doctor.

Doctor :   1. It has to be examined by a Specialist - Ortho for proper diagnosis of a complete or incomplete tear.

User :   If it is a complete tear, do I have to have surgery?
Doctor :   Yes, if a complete tear a suregery is mandatory , if incomplete a cast above the knee for six weeks and below the knee for six weeks.
Doctor :   A Plaster of Paris Cast.
Doctor :   As first aid I recommend you to follow the following steps : Doctor :   1. Use lots of ice packing.
Doctor :   2. Take NSAIDS like Advil tab. thrice daily 400mg. for 3 days.
Doctor :   3. Please use a Crepe band , and ask a primary to help you out.And try to restrict movements in the injured foot.

User :   On #1, I have an ice pack that my leg is resting on but it is too big to wrap it.
User :   On #2 -- the Advil is just for swelling, I guess? Or is it also for pain?
Doctor :   It both for pain & swelling
User :   On #3, I am getting a physician to wrap my foot later tonight.
Doctor :   And if 400mg doesnt work you can go ahead for 6oomg , nothing wrong.

User :   Sorry, just to check -- surgery is absolutely mandatory for a tear?
Doctor :   If at all it is complete then surgery is the best possible help.
Doctor :   It is called Early repair for Achilles tendon if done before one week of injury and has better recovery results.
