User :   GM doctor, my age is 24 and i have a scrotum absence problem which treatment shall i do ?
User :   hi
User :   Dr AAAAAAA here
User :   please describe your problem in detail.
User :   hi doctor , actully i have scrotum absenteisum problem i do treatement but doctor said its not curable now what i do?
User :   do you mean to say that testis are absent from scrotal sac?
User :   if yes is it on both sides or only one side?
User :   Yes, the all report is with me
User :   ok. what investigations have been done? what were the results?
User :   Like MRI, Blood Test ( Testosterone is very Low), Abdonmane
User :   THe MRI report say that No Evidence of any abnormal soft tissues nodule in the bilateral inguinoscrotal region
User :   and Abdonmane report is No testis seen on either side scrotum or inguinal canal
User :   Doctor their is no hair growth near testis & on leg also on face
User :   ok. first we need to find out if there is any testis present at all
User :   it wil need special scan to locate any testicular tissue along with blood teststerone levels and its precursors
User :   to see if testosterone production is going on fine
User :   are the hair development fine in axilla and pubic region?
User :   Yes but very rare the Doctor given me one Creame of Testosterone
User :   you might also need a Karyotyping test to look for any abnormality if testis tissue is not found at all.
User :   Shall i do this Karyotyping test
User :   yes, along with that you will also need to get blood teststerone levels and its precursors. discuss about getting these done by your treating doctor
User :   I Think I have this report it shown Blood testostrone , Serum by CLIP 31.04 instread on 241 to 827
User :   are you sure of the report? if yes then it is very low, you w need the precursor levels tested to see if production is fine or not. it is special test requiring prescription of your treating doctor
User :   Which test you told me special test
User :   the precursor tests like androstenedione and DHEA levels
User :   Actually this report from XXXX
User :   if these are also low then you will need Karyotyping test to be done to look for serious issues like any chromosomal abnormalities
User :   have you followed what I have said so far?
User :   Sir there is another report is shown Chromosomal Analysis and result is Karyotype 46, XY ( Normal Karyotype
User :   ok. are you sure of this report? since the MRI scan failed to show presence of testis in abdominal cavity, consider getting repeat chromosomal analysis/karyotyping
User :   not sure but this Chromosomal Anyalusis report with me sir
User :   ok. then request your treating doctor to evaluate for these : measurements of serum electrolytes, testosterone, müllerian-inhibiting hormone, and adrenal hormones and metabolites (17hydroxyprogesterone) levels
User :   testosterone, luteinizing hormone (LH), follicle-stimulating hormone (FSH) and müllerian-inhibiting substance
User :   you can also consider getting a laparoscopic exploration done to evaluate what is going on along with these.
User :   Report Analysis is No of Cells Counted 20, Analyzed 20 , Karyotyped 02 , Karyotype 46,xY Interpretaton , Normal Male Karyotype
User :   if they are found during exploration, a surgery called Orchioplexy can be done to get them into scrotum
User :   Shall i go with dircet to surgery
User :   Becuuse doctor said last saturday it's not curable if you do countinue this testosterone tretement it effect on your body & bone's
User :   getting these preliminary tests done before surgery will help to decide on prognosis with surgery
User :   ecuuse doctor said last saturday it's not curable if you do countinue this testosterone tretement it effect on your body & bone's
User :   yes, he is correct in telling that it will effect on your body & bone's .
User :   the question of curable or nor curable can be decided with these tests that I have mentioned along with laparoscopic exploration.
User :   Ok then Thanks a lot for your valuable suggestion & Time . I will meet tonight again doctor & Discuss the same Thanks a lot Again
User :   ok. is there anything else I can help you with?
