User :   I am here to discuss some questions I have about mild to moderate abdominal burning

User :   about 2 years, on and off, I know the exact week it started (July of 07)

User :   and has been on and off since

User :   yes, no xray or ultrasound but I had a sigmoidoscopy done because I noticed the pain is worse when I have gas or the bowels are full
User :   and usually feels better after I pass gas or a bowel movement
User :   but during this same period
Doctor :   is there any bowel difficulty
User :   bowel difficulty to some degree yes
User :   sphincter muscle seems to spasm a lot and has caused a fissure
User :   and now a thrombosed external pile
User :   some degree of constipation
Doctor :   any passing of blood or mucus in stool

User :   blood yes but has stopped after last month or so of treating the fissure
Doctor :   how is the pain
User :   they sigmoidoscopy came back normal
User :   mucus was only once
User :   and passed while passing gas
Doctor :   dull,sharp, cacthy, continous
User :   never saw that again

User :   it is locallized to about the size of a quarter
User :   burning and tightness
User :   but happens in 2 or 3 regions as shown in the picture
Doctor :   any previous history of urinary tract infection
User :   no but I noticed increased urination 3X or more over the past 2 years
Doctor :   does pain radiate to umbilicus
User :   last night I had to urinate several times passing a lot of urine
User :   not to umbilicus no
User :   I lost 5 lbs during the night
User :   and urinated a lot
Doctor :   ok, is pain associated with fever, fatiuge
User :   no fever
User :   but fatigue yes

User :   I sleept all day yesterday
User :   and sleeping a lot can sometimes cause constipation for me
User :   or even being tired
Doctor :   do you any pain, which may u unable to your routine work
User :   no in this pain I can function
User :   just nervous because it has been 2 years and still comes and goes

User :   cannot feel any lumps
Doctor :   ok, are you suspecting any type of tumor?
User :   not at this time
User :   I have had lots of blood work and urine tests done over the past 2 years, nothing abnormal yet
Doctor :   no, its not possible at this age
User :   I have more blood tests in progress including A1C for diabetees
User :   but not sure about this pain
Doctor :   ok, first thing is u can go for scan
Doctor :   any stones yu are thinking
User :   ok, what kind of scan would you advise?
User :   not sure about stones
Doctor :   like, renal or gal bladder
User :   Is this where I would feel pain if I had stones?
Doctor :   ultrasound abdomen
Doctor :   no the pain will be horrible
User :   ok, good ultrasound is easy and inexpensive, and no laxatives needed
Doctor :   you will be tossing in the bed if stones

User :   I don't think it is that severe
User :   only mild to moderate
User :   but very deffinate
Doctor :   yes, you csn walk into any diagnostic
Doctor :   yes, stones and appendicitis can be detected
User :   can appendicitis be chronic and intermittant over 2 years>

Doctor :   see very small stones can give moderate pain
Doctor :   yes , recurrent mild infection and chronic cases it can b

Doctor :   so, scan is the next best thing to do...
Doctor :   have any prostrate problem
User :   I don't think it is prostate, I have had several rectal exams in the past few months because of the sphincter muscle, pile and fissure and prostate was noted normal or not noted each time
User :   And I am young
Doctor :   hiatus hernia, gatritis, hormonal disorder can give mild pain
User :   but I have had major changes in urination
Doctor :   prostrate infection can occur any time
User :   I am on antibiotics now and prednesone
User :   for sinusitus
Doctor :   ok, could b chronic uti
Doctor :   since how many days u are on sterids
User :   1 week

User :   oh not sure, my doctor thinks I may have reflux
User :   I had 2 episodes of severe (non cardiovascular) chest pain in the past 2 months
Doctor :   ok, not to worry, when was the urine routine and culture done last?
Doctor :   have any skin lesions or allergic to sunlight?
User :   can't remember last the urine was done, hmm
User :   no skin lesions
User :   or issues with sunlight
Doctor :   any family history of abdoninal chroic pain
User :   3 uncles with diverticulisis, cousin with chrons
Doctor :   ok, porpyrias i was thinking, it is very remote
User :   never heard of it
Doctor :   ok, but you said no blood in the stools
User :   no blood in stool from upper GI
User :   I have had fecal occult tests on rectum but not on stool samples
Doctor :   may be asymtomatic diverticulits also???
User :   no visible blood in stool or what would be described as tarry
User :   good news is that no diverticula were found on my sigmoidoscopy
Doctor :   ok, simple is go for a scan, with the report we discus
User :   ok so ultrasound scan
User :   any other tests i should request?
Doctor :   ya
User :   urine?
Doctor :   urine routine and culture

User :   I will ask about both next dr visit
