User :   i wanted to know about thyroid problems in females
User :   i have a niece who is 15 and she has got tested for thyroid the result came as T3 NORMAL, T4 NORMAL, TSH was 9
Doctor :   May I know why was the test done?
Doctor :   I mean can you describe the symptoms that she had?
User :   this test was suggested by her regular physician, as she was having swelling in her fingers of hands and also her voice changes some times,
User :   when she is tired

User :   he suggested to give her thyronorm 25 mcg

Doctor :   Does she have history of thyroid problems in the past?

User :   but i stopped my sister as i felt there are a lot of differences in test results and we will recheck her thyroid after her periods and then take a decision

Doctor :   yes, she has hypothyroidism.
Doctor :   Hypothyroidism is characterised by high TSH values.
User :   but is that determined by just one test

Doctor :   and when T3 is normal, doctor suggest free T3 levels to confirm hypothyroidism.
Doctor :   My opinion is based on the reports alone.
User :   so u say we do not have to get it rechecked
User :   why i am asking u is, i know people who have had problems after taking thyronorm without proper guidance, i heard this hormone makes a lot of difference and has different effects on different people

User :   so without second opinion i did not want to start that drug on her as she is too young to be put on a hormone

Doctor :   Free T3 is the active form of thyroid hormone which is responsible for all the effects of on the body.
User :   ok
User :   so we should go to the lab and ask for thyroid test and free t3
Doctor :   May I know for how many days did she receive thyroid hormone supplements?'
User :   no sir, we did not give her any supplement
Doctor :   *thyronorm tablet, I meant?
User :   as we wanted to take second opinion and recheck her thyroid before starting this drug
User :   we did not give her the supplements at all

Doctor :   If she has not taken the thyronorm supplements, you need not wait for the second test,
Doctor :   You can ask for thyroid function test with freeT3 levels.
Doctor :   Some labs will include free t3 levels when doctors suggest for thyroid function test, however some may not, so you can specify it to them too.

Doctor :   If the second test shows high TSH values with or without low freeT3 levels, then it is suggestive of hypothyroidism.
Doctor :   She may need the hormone supplements.
Doctor :   And as far as the adverse effects of this hormone is considered,
Doctor :   Adverse effects is only if the hormones level exceed the normal levels.
Doctor :   She will need monitoring of the TSH values while on the supplements.
Doctor :   And as and when the TSH levels normalize, the medication will be tapered down.
User :   oh ok

User :   yes. i will get the test done but
User :   next time after the test results if i come back for a chat will it be the same doctor chatting with me or will i have to give the history again
