The medical condition that might want to know is Preeclampsia. The symptoms you describe are partly indicators of Preeclampsia. This is a pregnancy-related condition occurs usually after 24 weeks of pregnancy, in which your blood pressure goes up to dangerous levels, causes water retention [therefore the swelling], and high levels of protein excreted in your urine. If left untreated, it might progress to the potentially deadly Eclampsia, in which the fore mentioned symptoms worsen and the woman starts to experience seizures. The high blood pressure might cause the placenta to detach from the internal wall of the womb [placental abruption]; this will disrupt oxygen supply to the baby, ultimately causing its death if a cesarean-section isn't performed immediately. The mother's life can also be in danger because of the profuse bleeding associated to placental abruption, and/or the convulsions. Right now you should not bother if you are following correctly with the doctor.
