if by IDL you mean LDL cholesterol then 223 is very high and worrisome. With such high values treatment is indicated. You've got to reduce cholesterol intake and your body weight (if you're overweight) and increase your physical activity (if you're not already active). The aforementioned interventions may decrease it by 10-15% at most. A statin drug would decrease it by 35-50%.
