If I were your doctor, I would clean the entry wound with an antiseptic lotion, prescribe for you a tetanus toxoid if not vaccinated, get your Hepatitis B and HIV status done by blood tests, and then decide on vaccination against Hepatitis B. If the HIV status is equivocal, then wait for three months, and repeat the test again.

Not if it has never been used, or only used by you. If it is someone else's needle then you need to go the nearest urgent care center and have the needle's "owner" tested to make sure they have not got blood-borne diseases like HIV, hepatitis B hepatitis C, etc... If it was on the street etc... again, you should seek medical help since taking medicines may decrease your risk of getting HIV etc..
