Hi this may be due to infection or due to injury. Since swollen and red (suggest inflammation) needs to be evaluated and treat the problem ASAP. Better if you can upload a photo of the toe which will help to diagnose better with any other features like raise temperature locally or fever, crying, etc. For now give cold compression and can give acetaminophen syp based on body weight to reduce pain.
