Dr. lets go part by part . Hyperkalemia with low urine output most commonly seen in KIdney failure . So kindly test for Serum Creatinine and Blood Urea USG abdomen pelvis .
Same Kidney failure can cause Metabolic Acidosis or Pulmonary edema which can cause Breathlessness , so Get an CXR-PA , Arterial Blood GAs analysis .

Chestpain when coughing
There are a few possibilities. You may have pulled a muscle in your chest or bruised a rib or cartilage. You may have inflamed lungs if you have had a recent chest infection. I suggest you take paracetamol and ibuprofen for the pain. If the pain does not improve after 5 days, see your doctor.
