2. SentinelPile( Nothing to do with hemorrhoid, most likely to be due to chronic fissure in anus).

A good question. But there are no such known cases to have a clot from the piles to be embolizing to the heart. The probable answer is : Once the pile gets thrombosed, the clots gets strictly adherent to the vein and hence can not be dislodged.
Most of the cases of thrombosed pile may go into fibrosis and may not need a surgical treatment unless they are prolapsed also.
