Low mono has not much significance but the high alk phos suggests there is obstruction to the bile flow.

lactulose is basically to avoid hyper ammonemia provided to patients with liver diseases. it helps preventing hepatic encephalopathy. along with neomycin and spironolactone and rifaxamines. also prescribed in constipations and flatulences. Cheers Khan
