What To Do For Premature Ejaculation?

I am male, 61, 6’2”, 343#. I have been completely monogamous for more than 40 years. I have osteopenia, high cholesterol, high blood pressure, and low testosterone all controlled by medication. Stage 3 CKD, stabilized. Osteoarthritis & chronic gouty arthritis are problems in knees, hips and lower back. I do not use NSAIDs due to kidneys. I do not have diabetes (near perfect A1c & glucose tolerance). I am on long term Keflex due to chronic staff infections. Gradually (over past 2 years?) my pre-ejaculatory fluid has become extremely bitter to my wife and developed an anesthetic effect on her mouth or genitalia as well as my genitalia . This has become sufficient as to prevent climax except by masturbation, and then only with great care to avoid contact between my genitalia my pre-ejaculatory fluid. The flow of my pre-ejaculatory fluid is almost constant and has increased in volume to exceed that of my ejaculation. PSA is near below 2 and manual exam is normal. Any suggestions other than forgetting our sex life?

Premature ejaculation can be treated with an SSRI medication such as escitalopram or sertraline. These medications prolong the time to ejaculation. I would talk to your doctor about trying one of these medications.
