General Surgeon 's  Response
Hi.
Thanks for your query and an elucidate history.
I think you need to add metrogyl 44 thrice daily and clomin/ antispasmodic medicines ; and you will be fine.
Change over to cefixime 200 twice daily.

The pain in the rectum can be severe proctitis and gets well with medicines , you do not need to apply anything locally.

IF you do not get response in a few days , consult a surgeon / gastroenterologist as you need colonoscopy and other investigations.
IF you get a relief, continue the medicines for 5 days.
Take boiled and cooled water and diet rich in fibers.
