I have gone through your query and can understand your concerns..
As per your complain in case if you have a ruptured ear drum since long and has not healed yet then you need to take care of it and get it repaired surgically by getting
Tympanoplasty done that is a surgical repair of the
eardrum.
It is necessary as there can be loss of hearing..
Other thing is that if there is a pimple in the left ear that s commonly due to infection and as after taking antibiotics there is no pain it is a good sign and shows that resolution of infection is taking place.
In case if there is still clogged feeling in ear and and you are not able to hear properly, it can be due to fluid accumulation behind the eardrum or
blocked Eustachian tube leading to reduced hearing..
I would therefore suggest you to consult an Otolaryngologist and get evaluated and a thorough clinical evaluation and investigations like Otoscopy,
Audiometry,
x ray etc can help in diagnosis and treatment can be done accordingly..
In case if there is fluid you can be advised oral as well as nasal decongestants..
If it does not clear then surgical
drainage of ear can be done..
