Hello dear, thanks for your question on HCM. In my opinion you should consult pulmonologist for your mother and get done 1. Clinical examination of respiratory system. 2. Chest x ray. Since she is not improving with drugs and having worsening cough, fever and chest pain , possibility of lung infection ( pneumonia ) is more. Chest pain on coughing is characteristic feature of pleurisy ( inflammation of pleura ). And pneumonia is the commonest cause for pleurisy. So better to rule out pneumonia first. So chest x ray is needed for this. She may need antibiotic drugs too. So better to first diagnose herself and then start appropriate treatment.
