Diabetologist 's  Response
Hi, welcome to our site. I am Dr Saumya Mittal.

Read your query. That is a very significant question and i appreciate your problem. I will try my best to answer your query

I am sorry to say that the query is not completely clear.

First you said that you feel sick in left top side. Left top side of what? Stomach? Chest? Head?

Besides, you just told me what your doctors told you. It may not help me completely. I would prefer if you update all the reports you have. Thereby I can see for myself all the tests done so far, their reports and I can suggest more accurately what you need to get done further. For example, I would have suggested a blood culture. But in this case I am not sure if you have already had it done?

Third, are you a well controlled diabetic or are the sugars high?

Outsides of this, I summarize from your query that you are a diabetic on insulin. I am assuming you are well controlled diabetic. I am assuming that you have been told that you have inflammation on the basis of raised ESR. Therefore I may add that even diabetics have raised ESR.

I am assuming they are suspecting a chronic infection like TB. And therefore I suggest that you have cat scan of chest as well as abdomen. So a full scan can be done. Be careful if you are told to have contrast. Diabetics usually have hidden kidney disease and the dye will only make it worse. A MRI of abdomen and a cat scan of chest may thus be better.

I hope this helps you. Inform the reports mentioned above and all the details so i can be of help further. Best of luck.

I have given you the answer to the maximum considering the information provided. The results of the tests could further enhance my answer to you.

Please do understand that some details could be extracted from a detailed history and examination.

Looking forward to your return query with the details asked so that I can help you further.
