Hallow Madam,

Since natural attempts by normal marital relations have not yielded into pregnancy, you may initially go for certain investigations to ascertain the normalcy of your reproductory organs and function.

1. Ovulation: You can find this out by ovulation detection kit or Ultrasonography ovulation monitoring kit. I prefer the USG method as it provides additional information about the follicular size (18mm - 21 mm is the best) and the thickening of uterine lining (endometrium - 9 mm to 14 mm is most conducive) at the time of egg release. Histological study of endometrium, FSH, LH, Prolactin levels also are helpful to get the evidence of ovulation.

2. Patency of the Genital Tract: Laparoscopy and/or Hysterosalpingography (X-ray of internal genital organs with radio-opaque dye) are useful investigations.

3. Semen examination after 4 days sexual abstinence.

If all these show normalcy, you may plan your sexual intercourse at and around the day of ovulation keeping in mind that the egg has a life of 24 hours and the sperms are active for 72 hours. Ultrasonography ovulation monitoring will help you in this. Still if you fail to conceive, you may have to go for Artificial Reproductory Techniques.

I hope for initial management, this is sufficient information. For any more advice, you may ask me Direct question.
