Hi. It seems you have infection in your Semen leading to the decreased motility and abnormal forms. You should get a Semen culture done and take antibiotics according to the culture report. Yoy should take the antibiotics for 2 to 3 weeks and then repeat a Semen analysis after three months. This would definitely help your Semen quality. Take care.
