The dose of thyroid medicine is determined by monitoring the blood tests while on treatment. Unfortunately a standard dose cannot be calculated using weight and height measurements. Since you were controlled on 25 mcg in for awhile, I would start back on that dose. If your TSH was above normal recently and you were already taking 25 mcg, you might need to go to 50 mcg. Then you need to stay on the thyroid medicine all the time and get your levels checked periodically. Adjustments in the dose may need to be made from time to time after that. Hope this helps.
