Uterus is retroverted normally. It is the normal axial position of uterus. Uterus has three layers most inner one is endometrium which has cyclical changes with the different phases of menstrual cycle. Endometrial thickness beyond a limit is pathological and usually helps to detect in uterine cancers or conditions like dysfunctional uterine bleeding. In your report, the endometrial thickness is not mentioned. There is a blood clot present in the endometrium which means, it is in the last phase of the menstrual cycle and will soon start shedding and periodic flow may start. This kind of picture is also seen in conditions with delayed periods where endometrium becomes thick and filled with blood clots and later sheds off resulting in heavy bleeding. You have to correlate it clinically.
