hi
upper lobes fibrotic changes can be due to prolonged tubercular infection,
due to long term tubercular infection the gross volume of the lung reduces to get the chronic lung changes like fibrosis, increased markings, cavitatory lesions.
due to which the chest mass gets atrophied leading to wasting of the muscles, with reciprocative changes causing slight twist of the spine into thoracic spondylosis.
mild cardiomegaly can be due to any mild enlargement in the cardiac status, especially left ventricle, possible due to prolonged systemic problems.
atheromatous plague are fatty deposits on the aorta.
all the best, take care
