What Does This Below Blood Test Report Indicate?

i am 50 yrs old , man , diabetic on oral drugs

since 3 yrs I got pericardial effusion , which resolved partially and increased then i had it aspirated since 3 month , fleuid showed no malignancy , PCR for TB negative , no other microorganisms.

sinc one month i had my ESR elevated (130) which was basically 60-70 ist hour , Creative 6 ( n=0.6) , low grade fever , anemia , lymphocytopenia ( absolute no =800) , I did antinuclear AB which was -ve (9) , anti body titer was IgE=17 (n=1800)

Is it an infection or auto immumity , and what to do else??

Looking to your profile
you have very high ESR, anemia, low grade fever, creatinine 6 and pericardial effusion. If i would have been your treating clinician, i would have suggested few tests like Serum proteins, Urine proteins and Urine routine and micro examination and kidney function tests like creatinine and others.
It is necessary to exclude myeloma and follow accordingly.
