What Does This CT Scan Report Indicate?
My brother, 33 years old, has been suffering a headache for last 6 months. His BP is 160/120 yesterday. His CT scan report is as follows:
FINDINGS:
1.a) On pre & post contrast scans, a smoothly demarcated extraaxial cyst (4.1 x 4.8 cm) with thin wall is seen at posterior cranial fossa (at midline with bilateral para-median extension); and attenuation of which is similar to CSF. No enhancement is noticed on post contrast scans.
b) Vermis & cerebellum is compressed & displaced ateriorly by the cystic lesion. Anteriorly compressed/displaced (mild) vermis & cerebellum cause compression on 4th ventricle & aqueduct of Sylvius with resultant restriction in CSF channel with mild to moderate dilatation of the 3rd & lateral ventricles.
c) No gross dysgenesis vermis & cerebellum is evident.
2. No evidence of intra or extra axial hematoma, infarcts, abnormality in gray-white matter interface or in appearance of sulci and gyri in rest of the brain. No evidence of contralateral shifting of mid-line structures.
3. Pituitary region: No mass at para, supra or intraseller region.
IMPRESSION:
SOL at posterior cranial fossa with mild to moderate tri-ventricular obstructive hydrocephalus, suggestive of arachnoid cyst (4.1 x 4.8 cm).
Please inform me how serious the problem is? What is your suggestion for this?

Arachnoid cysts are not tumors but are cysts (fluid-filled sacs). Arachnoid cysts that do not cause symptoms or impact surrounding areas do not require treatment, no matter where they are located or how large they are. Otherwise, surgery is recommended. Since there is hydrocephalus due to the cyst therefore surgery may be considered by the neurosurgeon.
