Your semen analysis report suggests infection with decreased sperm count and excess abnormal forms of sperms.
Infection is indicated by the presence of WBC in semen.Normally there shouldn't be any WBC in semen.
Infection may be due to prostatitis,UTI etc.
You need antibiotics for infection.
You need few more investigation like routine hemogram,Semen culture and sensitivity,RBS,Ultrasound of pelvis should be done.Trans rectal sonography of prostate can be done if needed.
Proper antibiotics should be prescribed after culture and sensitivity report.
At least 16 %Sperms should have normal morphology.
Antiestrogens should be prescribed for abnormal morphology.
If normal morphology is persistently low,then you may need assisted method fertilization like intracytoplasmic sperm injection (ICSI).
Motility is acceptable.
