Hello

Pneumonitis indicates active infection in lungs where as fibrosis is sequel of past infection.
It is important to rule out infective etiology.Pneumonitis need antibiotics where as fibrosis doesn't need any treatment unless it compromises pulmonary functions.
You also need further tests like routine hemogram.
Your symptoms are more in favour of pneumonia but it need clinical correlation.
CT scan can be done if there is doubt.

Hope i have answered your query.
Take Care
