What Does Persistent High Grade Fever Indicate?

My son (age 28,weight 61kg,height 5 5 )had a high grade fever (around 104 degree F )for more than a week and he was hospitalised for 7 days where despite dengue,malaria and widal negative(all titres Stop,Sth,Spahn and Spbh less than 20) ,he responded to anti malarial treatment(along with Inj Artesunate and Tab Doxycyline also given Ceftriaxone 1 gm twice daily for 1 week.His Crp which was very high 137.1 mg/litre at the time of admission was 41.6 mg/litre. The fever relapsed again dafter 3 weeks but was low grade hovering around 100-101 degree F. WIDAL done again was less than 20 except Spbh showing 1:80 and he is responding to Oratil 500 (given for 5 days twice daily).ESR was normal 9 mm at 1 hour.Crp 8.65 mg/ litre.The physician wants to admin irse stronger antibiotic for another week and repeat WIDAL .IS he right?

I really appreciate your concern, if this confirmed case of malaria then it is such infection that reduces the immunity and then chances of opportunistic infection increase and it can be enteric fever, differential diagnosis need to be taken under consideration, like lung disease, cerebral infection, tuberculosis, collagen disease, if the physician is thinking for higher antibiotic then it is the right move hope this information helps, take care and have a nice day.
