Hi welcome to the health care magic

In your provided report rather then Morphology, motility affected more...

Normally sperm Morphology Normal in greater then 30% can be considered normal

Sperm motility should be atleast 40% and actively motile should be 32%...

In report infection is also mentioned, so there might be presence of pus cells also in semen..
Infection is one of the cause for affection of motility

So my advise is to complete course of antibiotic and after three day of sexual abstinence again collect semen in sterile container and repeat semen analysis

If still motility abnormal then we will discuss further causes

Hope I have solved your query

Take care
