What Does Inhomogenous Endometrium In The Ultrasound Report Suggest?

My ultrasound said: The uterus is midline and anteverted measuring approx 6.9 x 5.4 x 6.2 in dimension> The endometrium is mildly inhomogenous, measuring approx 15mm in combined thickness> No myometrial abnormality is detected. An anechoic tubular structure posterior to the right ovary measured 4mm, possibly representing mild hydrosalpinx. Moderate fluid is noted in the pouch of Douglas and left adnexa. Question: what is INHOMOGENOUS? WHAT IS HYDROSALPINX and what is the DOUGLAS and left ADNEXA? thanks for the assistance
