Hello and welcome to HCM,
Ileocaecal Koch's with mesenteric lymphadenitis implies tuberculosis of the ileo-caecal region with involvement of local lymph nodes.
ADA levels are elevated in ascitic fluid in cases of tuberculosis of the intestine.
Tuberculosis is caused by bacteria called Mycobacterium tuberculosis.
Treatment of intestinal tuberculosis is anti-tubercular treatment (ATT).
Thus, you need to consult your physician for prescription of ATT once ADA levels are done.

Thanks and take care
