What Does Circumferential Disc Bulge In The MRI Scan Suggest?

Hai,I am Ashok , i have a back pain. i had taken MRI scan. the details below mentioned.MRI Study of Lumbo - sacral Spine, Impression:1. Circumferential disc bulge with indentation of the anterior thecal sac is visualized at L2-L3 level.2. Large central and right paracentral posterior disc extrusion is visualized at L3-L4 level. The extruded disc material compresses the L4 nerves and right side- more on right side.3. Posterior disc protrusion with indentation of the anterior thecal sac is visualized at L4-L5 level. The protruded disc impinges the L5 nerves.4)The Vertebral bodies , pedicles, laminae , spinous process and transverse process of the lumber vertebrae appear normal.No Evidence of Spondylolysis.5)Facet Joint degenerative changes are visualized at lower lumber levels.The Bony Spinal Canal Diameter is NormalLevel      L1     L2     L3     L4     L5
Cm     1.5     1.4     1.3     1.1     1.0

The Spinal Cord , Conus medullaris cauda equina and the sub arachnoid space are normal. The paraspinal soft tissues appear normal.
The Sacroilliac joints appear normal.
Whole Spine Screening.
Posterior disc bulge is visualized at C3-C4 and C6-C7 Levels.
No Significant abnormality is seen in the dorsal spine.
Impression:

1. Circumferential disc bulge with indentation of the anterior thecal sac is visualized at L2-L3 level.
2. Large central and right paracentral posterior disc extrusion is visualized at L3-L4 level. The extruded disc material compresses the L4 nerves and right side- more on right side.
3. Posterior disc protrusion with indentation of the anterior thecal sac is visualized at L4-L5 level. The protruded disc impinges the L5 nerves.

Note :
Kindly suggest me That i have to go to under surgery.
Or It can be cured with the medicine .

Hello

Report suggests degenerative disc disease of lumbar spine.
Degenerative changes occurs in spine due to aging and development of weakness in annulus fibrosis.

1.At L2-3 level,there is circumferential disc bulge which is indenting the anterior thecal sac.

2.At L3-4 level,there is large central and right paracentral posterior disc herniation which is compressing L4 nerve roots(more in right side).

3.At L4-5 level,there is posterior disc herniation which is indenting the anterior thecal sac and it is impinging L5 nerve roots

Majority of the cases of degenerative spine disease are benefited by physiotherapy,so physiotherapy should be tried first.
Surgery should be the last option.

Treatment can be planned after consultation with neurologist.
