OBGYN, Maternal and Fetal Medicine 's  Response
Hi, I have gone through your question. Back Pain with cramps and tightening of the abdomen at 39 weeks pregnancy is suggestive of labour pains.
Traces of blood in urine at this gestation can be due to Labour pains, loss of mucus plug, or can be due to urinary tract infection.
I would suggest you to consult an OBGYN specialist so that you can be properly examined, investigated and treated accordingly.
Hope you found the answer helpful.
Wishing you good health.
