General & Family Physician 's  Response
Hello,
Welcome to HCM,
I understand your concern.
Your history suggests a Anal fistula, or fistula-in-ano, which is a chronic abnormal communication between the epithelialised surface of the anal canal and (usually) the perianal skin.
It could also be due to Boil or furuncle but they don't tend to last so long.
I understand you are shy to go to a doctor, but you condition warrents immediate medical attention. So I suggest dont delay any further and visit a surgeon, u may prefer to go to a lady surgeon.
Hopefully this information is useful to you.
Regards
