Welcome to Health care magic.

The report indicates there are some densities (white patches )in the right upper lobe which are not been able to asses exactly what they are - so doctor has asked for a apicolordotic view - in which the x-ray will be taken in such way that densities could be assessed.

In a 54 years old male with densities in the upper lobe - the possible cause could be any inflammatory / infection ( pneumonia ) , but according to the location - its most possible location for pulmonary tuberculosis. So for the same reason doctor has suspicion and ask for different view.

So please get it done, if sputum is there along with cough - a sputum test also might be needed.

Rest of your report appears normal - The trachea, heart, underlying bones and so on all with in normal limits.

Hope it helps you. Wish you a good health.
