General & Family Physician 's  Response
Hi, since you are hypertensive, over weight with type 2 diabetes, severe pain behind ear with nausea should be evaluated.
Take your medications regularly and monitor the sugar and BP levels.
Get cardiac evaluation and lipid profile once to rule out any other cause/problem.

If any abnormal get back to us with report for further discussion for management. If no problem detected then adhere to all your prescribed medications with diet and regular exercises.

Regards
