welcome to Health care magic.
1.Ischemic foci means, There is no blood supply to anterior - lateral part of the brain area (in your report).
2.It happens when a blood clot / plaque (calcified), travels from some other place and lodges in the brain vessel ( narrowed / stenosed ) causing blockage.
3.Happens in patients with hypertension and diabetic patients and patients with blood pathologies.
4.It can be treated by thrmbolysis therapy, and treating the underlying medical condition ( Controlled diabetes, hypertension )

Hope it helps you. Wish you a good health.
