What Causes Urinary Incontinence In An Elderly Person?

I am 74 years of age and 2 years ago I had a TURP operation. 2 months prior to the TURP I had my appendix removed. After the operation I couldn't pass urine, and left hospital with a catheter. 2 days later returned to hospital with blood in my urine and a bladder infection. Following the TURP procedure I had another more serious infection and had 6 months of incontinence, and a 3 month antibiotic treatment, but my penis had lost 3 inches in length. It has continued to shrink and has no response to anything. It is now so short I cannot use a urinal. Prior to the TURP I had normal function.
Twenty years ago I began to get hydroceles, they over time enlarged and stopped growing.
Lately the one on the right has enlarged very quickly, but still feels of soft fluid.
