What Causes Tiredness, Nausea, Dark Urine, Abdominal Pain And Fever?

CASE 1: A new patient presents to your office complaining of feeling tired and achy. He states that he is usually a good eater, but lately he feels bloated and easily nauseated. Upon further questioning he tells you he’s noticed his urine has been darker recently, but he hasn’t had much of an appetite for water. He hasn’t had blood work done in over a year. On physical exam, the right upper quadrant of his abdomen is tender to the touch and he has fever of 99.8. He is a 54 yo construction worker, non-smoker; he describes work as the ‘good sort of stress that he is used to’ and he lives alone, but likes to s pend time with his brother on the weekends watching football. What tests would you follow up with on this patient, and what do you suspect the patient has?
