Following are the causes of your problem:
Friction -
Frenulum injury - when the sex is already too hard, the frenulum may be ripped or torn, resulting in bleeding.
Balanitis - caused by poor hygiene and infection; that's why uncircumcised men are more prone to getting this condition. It is characterized by an inflammation of the penile head and the foreskin
Other causes - Genital piercings and an
allergy to latex condoms have also been reported to cause a cut on the penis.

Treatment for Penis Tears
Managing penis tears depends on its cause.
1.. Men are encouraged to insert the penis when the partner is already "wet" or at least has sufficient artificial lubrication applied. Lubricants aid in lessening friction to avoid injury for both parties. The to and fro motion may also contribute to the injury when the skin of the penis is dragged quickly.
2. For frenulum injury,
frenuloplasty, and other minor surgeries can be performed on short and thin frenulums.

Steroids and antibiotics are prescribed for balanitis
4. Antifungal creams are used to fight against
yeast infections.
5. Check your partner test also for sexual transmitted disease
Take following advice:
1.Change to a simple, unperfumed soap.
2.Alternatively, buy ‘aqueous cream’ from a pharmacy and use it as body-wash cream (i.e. apply a small amount and rinse off).
3. Put two handfuls of salt in the bath, but no other additives
4. Do not use ‘biological’ or ‘enzyme’ powders when washing your underpants
5. Showering after a workout, changing underwear, using a clean towel to dry off with and not redressing until the area is completely dry
6. Use ormulated penis vitamin formula- Man 1 Man Oil-contain:
