General & Family Physician 's  Response
Hi! I suggest that you need to get your blood pressure checked and your blood sugar levels determined, at least once. I would also request you to test your CD4 blood levels, since you are HIV positive. Swelling of the legs is called as pedal edema, and since it has a large number of different causes, the above three tests will determine what the issue really is. These tests are cheap and easily available and you do not need to get admitted to a hospital for them. If you want to get rid of the swelling, you will have to visit a doctor, who will prescribe you a diuretic tablet that will quickly get rid of the swelling, however, you need a prescription from a doctor for this, and since your health is important, you might want to see a local physician for this.

I hope this gives you the information you need.
All the best,
