Hi. It appears that you have a bad case of acute pharyngotonsillitis and it needs to be treated systematically. I would recommend that you have yourself re-evaluated by an ENT physician to examine your throat and to rule out some other conditions that can cause severe throat pain and difficulty in swallowing. Neck needs to be examined for lymphadenopathy and blood work for cell counts is indicated given the 4 day long persistence of symptoms. You will probably need a 5 day course of antibiotics which needs to be taken without interruption. I understand your condition is painful but you need to take antibiotics for at least 2-3 days before concluding on its efficacy. From the history it appears that you took it for just one day which is very inadequate. In addition to the antibiotics, a good anti-inflammatory tablet twice daily for 5 days that reduces swelling such as a paracetamol/diclofenac and serratiopeptidase combination (Zerodol SP) (You may dissolve the tablets in water if it is hard to swallow them) along with medicated gargles such as with betadine/chlorhexidine/Tantum will need to be taken before and after every meal for 5 days. It is recommended that you keep yourself hydrated by drinking plenty of fluids (Approx. 2 litres/ day) and have a soft bland diet till the symptoms recover. Getting admitted in the hospital for IV medications and IV fluids can be a possibility if your symptoms worsen or remain severe even after 48 hours of oral medications. I will let your ENT physician make the call on that decision.
