What Causes Severe Tooth Pain After Root Canal Procedure?

I had a root canal done on my 1st upper molar. Immediately after the pain meds wore off I had terrible pain
above the tooth area and it felt like there was an air apace above the upper gum under my right eye and very close to my nose. I have been on antibiotics for three weeks waiting for the pain and little swelling to subside. have not had the permanent crown done yet due to the pain. I don't know what to do. I can't put anything warmer than room temp in my mouth or the throbbing pain is horrible. I then have to put cold water and swish it around to calm down the pain. I have had sinus problems for years and the tooth right next to this one had a root canal years ago and has a crown. My last molar on that side has an implant now due to a failed RC. I don't know if I should go to a different dentist or an Ears, Nose and throat Dr. this severe pain started after the RC. The dentist said the tooth was dying and needed it. It did have symptoms of pain when chewing hard items. I have a very high tolerance for pain but I'm taking ibu every 6 hrs to dull the annoying throbbing. Can you suggest help please?

Can understand your concern..

As per your complain it seems that if you have pain just above the tooth in which root canal treatment is recently done it clearly indicates that you have an infection beneath the root canal treated tooth causing pus formation and the pus is getting accumulated in the soft tissues causing swelling..

As the feeling of air space can also be due to infection and pus formation causing swelling but if the swelling or air space feeling is extending from the region of the painful tooth it can be due to the tooth and if it is only in the region close to eye near the nose it can be due to sinus infection and inflammation of sinus lining..

One more thing that has to be considered is that the sinus infection can also occur due to infected tooth as the roots of molars are located very close to the sinus floor and any infection can lead to infection in sinus also..

I would suggest you to consult an ENDODONTIST and get evaluated and in case if the tooth is getting infected root canal filling has to be removed followed by antibiotics and painkillers to relieve the infection and once the infection and pain completely subsides you can get the treatment re done..

In case of sinus infection an ENT expert has to be consulted and requires antibiotics,painkillers and decongestants for the treatment of sinusitis..
