What Causes Severe Persistent Seborrheic Dermatitis?

Been diagnosed with sebborheic dermatitis via dermatologist and been using ketaconazole cream 2% and hydrocortisone cream 2x daily for 2 weeks. Has helped some but still flaring. Primarily on face, ears and a little in hairline. Totally not comfortable being out in the public. In other words the lesions are still present. A Nurse Practitioner that is close to the family said the next step would be fluconazole. What is the usual dosage for Fluconazole. Will ask the dermatologist if an oral antifungal can be prescribed.
