General & Family Physician 's  Response
Hi,

Severe lower abdominal pain along with vaginal discharge definitely warrants an examination by a physician. Probably you are having an infection in your cervix/uterus.
If I was your physician I would definitely do an ultrasound of the uterus and would culture the discharge (to see what it grows). I would also give you a course of antibiotics (7 to 10 days).
As for the blood clot, you mentioned you are taking birth control pills, these pills can affect the lining of the uterus resulting in abnormal vaginal bleeding/clot.

I wish you good health and an early recovery.
If you have further questions please feel free to contact me.

Best regards.
