What Causes Severe Pain In Shoulder Blade Radiating To The Fingers?

So two years agoish I fell down the stairs i threw my right arm out the stop myself from hitting the floor with my face. I held on to the rail with my left arm. Hit my shins. Went into physical therapy for 4 months and felt better doctor and physical therapist said go back to work full time. I went back with their blessing at full time plus some. I worked in laundry, a lot of repetitive movement. I started to feel major pain in my shoulder down the shoulder blade down the arm into my hand and into my pinky and forefinger a little into the neck around the shoulder blade into the underarm. If I don t take my medication it hurts to breath. I have full range of motion but its painful to move it, it is painful to walk. They can not find the reason and it is very frustrating. There have been MRI s, X-RAY s, EMG s, Ultrasounds of the veins since it swelled up after a horrible trip on a plane to go for a IME. The only thing they have been able to determine is that they believe I am in pain. The X-RAY shows where my shoulder bone has grown a little bit and the joint is not in there right, but they don t want to go in and shave down the bone until they figure out the rest of the problem.

thank you for providing the history of you.

A thorough neuromuscular assessment is advised.

By your history it appears to me as the cervical spine degenerative disc disease, in this , the disc provides the compression on the nerve root and the pain travels from the neck to shoulder upto the fingers depending upon the pinch on the nerve. Also, due to your repetitive movements in your job profile, the trapezius muscle will be spasmodic as by which you are having pain in the insertion of the trapezius muscle.

I will recommend to undergo physical therapy again. First you need to take therapeutic ultrasound therapy, TENS therapy, Myofascial release. By this, the pain, inflammation will be reduced in the muscles.

Later stages performing strengthening exercises for the muscles of the neck, shoulder and Trapezius will be of importance to control the symptoms and also, avoid the future symptoms.

You may even not need any surgery in future if you can succeed with physical therapy. In my clinical practice i see cases with similar complains and all respond well to physical therapy in 2-3 weeks of time.
