What Causes Severe Headache And Pressure In Head

My husband has been concussed twice during the last month playing rugby. he has had stitches twice, once upper eyelid and secondly on his temple. He has now had an excruciating headache for 4 days. no painkillers are working. he says the pressure is unbearable, and feels like his brain is going to explode!! please help - could this be very serious? he is also suffering from a cold at the moment, and hi nose is very blocked

Hi, thanks for asking from HCM.


I can understand your concern. Your husband is having severe headache which is not responding to painkiller. It should be taken seriously.

You must consult ER as soon as possible. NCCT head must be done urgently to rule out intracranial bleed or skull bone fracture. Further treatment will depend upon the diagnosis. If there will be small bleed, it will be managed conservatively.
If there will be large bleed or mass effect or midline shift, surgical evacuation might be needed.


You should also observe him for
- Vomiting
- Seizures
- Weakness in limbs
- Progressively worsening headache which is not responding to painkillers
- Altered consciousness or loss of consciousness for more than 5 minutes
- Spontaneous bleed through ear, nose or throat without injury.

If any of these signs is present, it is a red flag sign. You must hurry.

Another less dangerous possibility is " post-concussion syndrome". It can be suspected if NCCT head is normal.
In such case, you can ask him

- Keep head at higher level than chest

- Avoid any sedating drug

- Take painkiller like Tylenol or diclofenac for few days after prescription. Headache increases on forward bending due to increased blood supply in scalp and increased swelling. So head elevation is preferred.

- If no relief, you can ask doctor to him injectable painkiller like Paracetamol/diclofenac.

- Avoid pressure at pain site

- Give him steam to open blocked nose.
