General & Family Physician 's  Response
dear friend

both the conditions diabetes and hypertension cause major effect of arteries by accelerating atherosclerosis and narrowing the lumen of arteries, this reduces the blood supply to organ and there is gradual slow destruction of the organ or atrophy, to explain you the condition in lay-man's language,
brain has enough reserve to continuing to function normally in-spite of atrophy. if your father is not having any memory and cognitive function issues please discuss with your doctor and try options which increase blood supply to the brain and delay the progress, consultation a good neurologist, diabetologist and a physician will be in order to effectively manage the condition.
if the condition is allowed to progress unchecked it will ultimately result in senile/dementia.
