My granddaughter is in the ER with severe stomach pains. She has had these pains for quite a few weeks. Now she is vomitting green bile, has not had anything to eat in 24 hours as she can t keep anything down. She is also dehydrated. Doctors said to follow up with a gastroenterologist. She cannot stop vomitting etc and they want to send her home. this doesn t make any sense to me. She graduates in 5 days and can t even stand up. What could be the problem as the ER doctors don t seem to be doing anything to help her.
If such patient comes to my clinic with severe adominal pain and persistant vomitus i will ask if she has passed stools or flatus? Then i would avise a list of tests Ultrasound abdmn Xray erect abdmn Serum amylase Full blood count The possibility of intestinal obstruction is likely. I would kept her nil by mouth I.v fluid I.v antibiotics Pain killers Hope she would improve with conservative management,if not surgery is next option. It is better to consult general surgeon in your locality
